This facial will improve the appearance of the outermost areas of the vagina through exfoliation, steaming, and hydration-all to maintain a healthy, rejuvenated vaginal area. Vajacials are recommended for every woman that shaves their pubic area, regularly gets a Brazilian or bikini wax,
or simply wants to keep their intimate parts neat and tidy.

I understand that vajacial treatments may have positive or negative side effects as a result of doing a
vajacial treatment. I accept legal responsibility for my choice to do a vajacial treatment and waive
the responsibility of the practitioner and the facility where the vajacial treatment takes place in the
case that any of the named side effects (or others) may occur. (Irritation, rash, or inflammation).
A refund of the deposit will not be issued for any cancellations made less than 24 hours in advance.

For skin lightening treatments:
Intimate area lightening is a process of bleaching hyperpigmented (darker) skin to create an even skin tone.
Before coming in for the service, make sure to have no hair in the area to be treated. You can shave or wax two days prior to your appointment.
Please read and acknowledge the statements below:
- I acknowledge that the results of skin lightening vary and that no guarantees of specific results are offered or implied.
- I understand that if I experience any pain or discomfort during any session I will immediately inform the practitioner so that the treatment may be adjusted or terminated.
- I understand that intimate skin lightening should not be performed under certain medical conditions, including, but not limited to: active herpes simplex outbreak, active HPV infection, history of keloids, warts; use accutane; other transmittable diseases, irritated, raw, or broken skin, or if I am currently undergoing chemotherapy or other radioactive therapies.
- I agree to keep the practitioner updated as to any changes in my medical profile and understand
that there shall be no liability on the practitioner’s part should I fail to do so.
- I understand that after the treatment, the skin may appear pink or red, and/or feel irritated.
- I understand that risks associated with skin lightening treatment include, but are not limited to an allergic reaction, reactivation of herpes simplex virus outbreak, blisters, burning, redness, irritation, and/or scarring.
- I understand that V-Juvenate Spa will not refund or credit any amount of money because of my unhappiness with my final results.
- I understand that it may take multiple sessions to reach desired results
- I agree to hold V-Juvenate Spa and all employees, providers, medical directors, officers, directors, owners, and associates or authorized representatives harmless from any liability involved in the use of this treatment.
I certify that I have read this entire document and that I agree to all its provisions. I certify that I have had the opportunity to ask my questions regarding the procedure my questions have been answered satisfactorily. I fully understand the treatment conditions, the procedure, and possible side effects and I accept the risks. I hereby give my consent and authorization and release this establishment and its agents from any and all liability associated with the procedure.
